A company is looking for a Transitional Care Specialist to
enhance patient care and support care management processes.
Key Responsibilities
Engage patients post-discharge and coordinate follow-up care
Identify care gaps and request records from specialists for
continuity of care
Collaborate with multidisciplinary teams to provide optimal patient
outcomes
Required Qualifications, Training, and Education
3+ years of experience in a medical office or healthcare
setting
2+ years of experience as an LPN or MA
Experience with telephonic communication regarding patient care
needs
Comfort with technology, including Microsoft Office and electronic
health records
Ability to adapt in a dynamic and ambiguous environment
