Job Description:
A company is looking for a Care Manager- Remote.
Key Responsibilities
Engage with members and providers to develop and implement
comprehensive care plans
Assess ongoing care needs and adjust plans based on member progress
and changes in health status
Coordinate patient education and facilitate referrals to relevant
resources
Required Qualifications
Current multi-state compact Registered Nurse licensure; BSN
preferred
3-5 years of clinical experience, particularly in telephonic Case
Management
Training in motivational interviewing preferred
Proficiency with electronic medical records and Microsoft Office
applications
Ability to work from a home office that meets HIPAA
requirements
