Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate care management activities for members with physical
health needs.
Key Responsibilities
Evaluate member needs and barriers to care, recommending plans for
optimal outcomes
Develop and monitor care plans while collaborating with providers
and community resources
Provide education to members and families on healthcare processes
and available resources
Required Qualifications
Degree from an Accredited School of Nursing or Bachelor's degree in
Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
