Job Description:
A company is looking for a Remote HARP LTSS Care Manager.
Key Responsibilities
Advocates for and educates beneficiaries on services and health
benefits
Conducts assessments and develops care plans in alignment with
treatment goals
Coordinates with healthcare team members to ensure care plan goals
are achieved
Required Qualifications
NYS RN or LCSW, LMSW, LMFT, LMHC, LPC, or licensed psychologist
(any state)
Experience managing member information in a shared network
environment
Knowledge of community health practices for frail adults and
cognitively impaired seniors
Relevant work experience preferably as a Care Manager
Ability to manage large caseloads in a fast-paced environment
