Job Description:
A company is looking for a Case Manager, RN to provide
comprehensive telephonic case management services for patients with
emerging health risks.
Key Responsibilities
Perform initial and ongoing health risk assessments and collaborate
with patients to develop healthcare goals
Support patients transitioning from hospital to senior housing and
implement transitional care interventions
Monitor remote patient data and communicate health updates to
providers
Required Qualifications
Active unrestricted compact RN license; BSN preferred
2+ years of nursing experience in various care settings
Case Management experience strongly preferred
Certified Case Manager Certification preferred
Transition of care experience desired
